If you are considering mohs surgery, it is sensible to ask about the drawbacks as well as the benefits. This matters because the right treatment is not just about cure rates. It is also about time, recovery, wound size, and whether the procedure actually suits your specific skin cancer.

It takes longer than many people expect

One of the clearest disadvantages is that mohs surgery usually takes longer than a standard excision. The British Association of Dermatologists says it takes half a day on average, and the process can extend because each stage has to be examined under the microscope before the next decision is made.

Because mohs surgery is done in stages, the day can feel like a stop-start rather than straightforward. You may only spend a short time having tissue removed, then wait while the sample is processed and reviewed. That can make the appointment feel tiring, especially if you are already anxious or if the treated area is on the face.

It still carries the normal risks of skin surgery

Like any procedure, surgery still comes with surgical risks. Authoritative patient guidance lists bleeding, bruising, pain afterwards, infection, scarring, and temporary or permanent nerve damage that can lead to numbness, and in rare cases muscle weakness.

That is why mohs surgery should be seen as precise rather than risk-free. It improves margin control and helps spare healthy tissue, but it does not remove the normal realities of wound healing. You can still end up with swelling, tenderness, or a scar that takes time to settle.

The wound can be larger than you expected

A further drawback of surgery is uncertainty about the final wound size. Skin cancer often extends beyond what is visible on the surface, so the area that needs removing can be bigger than the original spot appeared to be. That can come as a surprise, particularly when the lesion is on the nose, eyelid, lip, or another very visible area.

After surgery, the repair may also be more involved than some patients first imagine. Depending on the final wound, closure may need straightforward stitches, a flap, a graft, or occasionally a longer healing process. Recovery advice from Cambridge University Hospitals also notes that patients usually need to avoid strenuous exercise, bending, straining, swimming, or heavy lifting for at least 14 days, which can be inconvenient if you are trying to plan work or daily responsibilities.

It is not automatically the right treatment for every case

Another disadvantage is that mohs surgery can sound like the best option for everyone when it is not. The BAD makes this point clearly: many skin cancers can be removed with standard excision and still have good outcomes. Mohs is usually reserved for cases where its extra precision offers a real benefit, such as poorly defined tumours, recurrent cancers, or high-risk sites where preserving healthy tissue matters more.

In some situations, mohs surgery may add time, waiting, and complexity without enough extra value. If a straightforward lesion in a less visible area can be treated effectively with a simpler approach, a standard excision may be entirely appropriate. That is why the decision should be based on the type of skin cancer, its location, whether it has been treated before, and what kind of reconstruction is likely afterwards.

The balanced view matters most

So, what are the disadvantages of mohs surgery? It can take longer, feel more tiring on the day, still carry the usual risks of skin surgery, leave a larger wound than expected, and turn out to be more treatment than some cases actually need. Those are real drawbacks, and they are worth understanding before you decide.

At the same time, these disadvantages need context. Mohs is used because, in the right case, its precision can offer a better chance of clearing the cancer while preserving more healthy tissue.
